Courses and Recurring Events
This form is to be completed by individuals seeking to reserve teaching space in the Health Sciences Center for courses and repeatable events.

Breakout Sessions
This form is used to request space assignements for small group sessions associated with larger lectures. It constitutes the second form required to book requests for courses which follow the lecture/small group discussion format.

Exams, Special Sessions & Events
This form is used for exams, one-time events, Grand Rounds, special administrative, faculty and student meetings, symposiums, and demonstrations.
