AV EQUIPMENT LOAN-OUT

Media Service’s AV Unit maintains a centralized pool of equipment available to faculty/staff for instructional support of classes, programs, and events. Additionally, we maintain a separate pool of equipment for the School of Medicine accessible to medical students, residents, and fellows.
Equipment may be borrowed on a daily basis and must be returned by 5 PM. Overnight or weekend loans must be returned by 8:30 AM on the next business day after use. Extended loans must be approved by the AV supervisor.
Borrowed equipment may only be used for academic or organizational activities of the HSC.
Please allow sufficient time before and after your scheduled class or event for verification of equipment operation at check-out/in (e.g. the laptop check-out/in process will take 5 minutes).
All reservations for AV equipment should be made in person at the AV office at least 24 hours in advance to assure availability. Forms may be downloaded from our web site and faxed to 4-3455. Please follow-up all faxed requests with a phone call to confirm receipt.
All requests for equipment and services require designation of a budget account and an authorizing signature. This information assists us in tracking equipment use patterns.
SOM student equipment request forms require an authorizing signature and account number from the sponsoring department.
